Free physicals for athletes from all area high schools contracted with McLeod Sports Medicine are available annually. The athletes must be rising 9th through 12th graders for the upcoming sports seasons. The one-day physicals are held at the McLeod Health and Fitness Center, 2437 Willwood Drive, Florence, SC.

Please print and complete the appropriate form below. The athlete must bring this form with them to the physical. Form must be signed by a parent in order for physical to be completed.
